Air/bone gap

The AB gaps at 500 Hz through 4000 Hz are certainly not uncommon. Of course,
the AB gap at 250 Hz of 90 dB would probably be artifact. As far as I know,
a 60 dB AB gap is the limit. But my next question is how did she use insert
earphones on someone who does not have an ear canal?
