I recently had a similar problem with a different text that was out of 
print.  I emailed the author and he said that he still retained the 
copyright on the book.  He gave me written permission to have the book 
photocopied if I remitted $4 for each book sold to him.  Our bookstore 
will actually take care of the photocopying and coil-binding if 
copyright permission is granted.  It is a pretty good option when a 
book is out of print.
